Player Registration Procedure
For competitive teams, you will receive a telephone call from the Coach the week following tryouts advising whether or not a player will be offered a place on a Real Madrid – Miami team. If your child accepts a spot on the team and is new to the club, it is MANDATORY that you attend the meeting called by the Team Manager to fill out all applicable registration forms.
Registration Meetings will be held during the first week of practice following try-outs.
Important documents to bring to the Registration Meeting or provide to your team manager:
1. Check made payable to SAA – Real Madrid Miami for payment.
2. 2 passport size photographs.
3. Copy of birth certificate or passport for new players
4. Notarized Medical Release
5. Any new player who is 12 years old or older, with a foreign birth certificate, who was not previously registered with FYSA is required to obtain international clearance. To do this you must complete the International Clearance Form and return it with a check in the amount of $10 made payable to USSF. If you have done this previously there is no need to do it again. (ask your Team Manager for this form).
